New Wiltshire 18 to 25 Moving On Service

Introduction

Currently the Children and Young People’s Disabilities Team (CYPDT) work with people who require social care support up to the age of 25.

Individuals transfer into adult social care services when they have completed their transition from children to adult provision.

Following an analysis of the current services and feedback received from parents and carers, it’s clear that the current process is not working. A number of concerns have been identified including: -

• Young adults are not being prepared adequately for adult services and are therefore anxious about the prospect

• High level of restrictive packages of care

• Early discussions with young adults and carers are not occurring

• Feedback about transitions are consistently poor

• Transfers from CYPDT to adult social care are delayed, resulting in poor outcomes

• Not enough is known about what to expect from adult social care

18 to 25 Moving On Service

In order to address this, a new service called the 18 to 25 Moving On Service will be established to work with people aged 18-25 who are eligible for support from Wiltshire Council social care.

The service will work alongside people aged 18, their families and their allocated worker to ensure a smooth transition from children’s to adult services.

Initially the service will work with people who are currently open to CYPDT, but over the next 18-24 months, it will expand to work with all people aged 18-24 who are eligible for support from Adult Social Care.

It is hoped that the benefits of this approach will include: -

• Young adults and their families are better prepared for adult service

• The 18 to 25 Moving on Service will be on the same system as other adult services. This will improve communication when individuals are transferring

• The service will work closely with the SEND Preparation for Adults Service, which will improve communication between the services

• Staff working in 18 to 25 Moving on Service will develop the knowledge and expertise to support people to have positive experiences of moving from children to adult services

• Packages of care will be less restrictive and more person-centred, resulting in better outcomes for people

As part of the development of the 18 to 25 Moving On Service, Wiltshire Council is undertaking to engage with parent carers to explore and discuss the proposal in partnership with Wiltshire Parent Carer Council and Carers Support Wiltshire.
